I declare that:
1. The information I have given is true and correct.
2. I am fit and able to take part in CCC activities and can swim 50 metres.
3. Whenever I take part in any CCC activities
- Any disabilities, previous injuries, allergies and other medical conditions that may impact my ability to take part in CCC activity or which may arise whilst taking part in CCC activity will be reported to the event organiser or river leader
- Any medication required to be taken, the event organiser or river leader must be made aware of it.
- If a parent or guardian (Chaperone), they must provide their emergency contact information to the event organiser or to the river leader on that day
4. I will inform the membership secretary (membership@cheltenhamcanoeclub.com) of any change in this information or of any change to the emergency contact information stated above.
5. I have read and agree to abide by the Club’s Code of Conduct on the Club’s website www.cheltenhamcanoeclub.co.uk
6. I understand that:
- canoeing and kayaking are “Assumed risk” “Water contact sports” that may carry attendant risks. Participants should be aware of and accept these risks and be re-sponsible for their own action and involvement.
- CCC accepts no responsibility for loss, damage or injury caused by or during attendance at any CCC organised activities
